According to tradition, the altar was transferred from St. Agnes Church on Stradom in Cracow. It was built in the 18th century. During its transfer, the altar was partially altered (gates were added). Then it was renovated around 1860. The main area of the altar contains the image of Madonna and the Child, probably from the 17th century (noted in the visitation carried out in 1730), while the altar final contains St. Nicholas' painting. At the sides of the altar, there are sculptures of the most important saints of the Dominicans and the Franciscan order.
